Position Summary
The Case Manager oversees the transitional process of each student entering and leaving the Institute. Responsibilities involve facilitating the case management process which includes leading the Treatment Team, developing Treatment Plans, evaluating student needs and ensuring the timely delivery of appropriate program services to meet these needs within the established guidelines of the Personal Growth Model. The Case Manager also serves as a liaison between the parents and the Program and between the appropriate State Agency and the Program.
Essential Job Duties
Oversees completion of a family assessment on all students within 7 days of enrollment; ensures individual treatment plan is developed within 14 days of the studentsâ arrival
Ensures all students have received academ! ic, vocational, and behavioral assessments within 14 days of e! nrollment and that this data is used in development of the individual treatment plan
Facilitates student progress meetings, with members of the treatment team monthly, or more frequently if needed
Ensures the disbursement of comprehensive, professionally-prepared monthly reports to the parent/guardian, judge and DJJ Probation Officer on each student within 2 business days of the Treatment Team meeting
Ensures advisors are completing weekly behavioral up-dates and weekly goal progressions
Attends monthly reviews and conducts monthly audits to ensure all pertinent information has been recorded once completed
Conducts home visits with studentâs families in accordance with program and state requirements, or as needed
Maintains case management notes and JJIS updates and alerts in accordance with established policy and standards to ensure adequate case treatment and documentation practices and reports discrepancies to superv! isor
May facilitate or assist with facilitation of ART, CYT and/or Life Skills group programs
Schedules transitional meetings 60 days prior to release date and assists in the development of a Transition Plan 30 days prior to the studentâs termination
Ensures each student is post-tested and has a completed transitional plan and transcript before release date
Conduct an exit interview with each student and parent/guardian; maintain evaluation forms completed and conduct aftercare calls in accordance with state and local requirements
Coordinates and/or provides all transportation for students - schedules off-site Court and Medical appointments; prepares referrals for off-site clinical services not provided by the institute; maintains reporting of out-service agencies; schedules motivational activities such as field trips and community service opportunities for youth.
Attend and professionally represent the program at cour! t hearings, meetings and student legal proceedings
Assists in ! establishing desirable student conduct and discipline; prevents and intervenes in student altercations and incidents
Leads and/or participates in positive reinforcement, interdisciplinary and/or care review meetings
Participates in programs and special activities involving facility and equipment cleaning, grounds maintenance and equipment relocation; operates institute vehicles as required in accordance with van policies and procedures.
Attends all required training/professional development events and maintains all appropriate certifications and licenses; cross-train staff to administer meetings in the absence of the Case Manager.
Assist with special projects and other duties as needed

Minimum Education, Training and Experience
Bachelor degree in Psychology, Social work or related field
One to two years related experience and/or training, preferably within Juvenile Justice or Family Services
Certifications, Licenses and Registrations
Must satisfy criminal background check and drug screening as defined by state requirements.
Current certification as a CPR and First Aid by a nationally recognized organization (e.g., American Heart Association)
Must possess and maintain a current valid driverâs license in the state of employment (CDL may be required with State specified endorsement).
Must satisfy criminal background check, reference checks and drug screening per AMIkids and state requirements.
Successfully complete and pass the physical restraint training course.
Be able to complete trainings outside the program (travel within the state) when required.
